New Patient Forms
New patient paperwork will be filled out online by clicking on the green New Patient Scheduling box on our webpage. Once you have filled out the paperwork you will use the submit button at the bottom of the paperwork to send it to our office. Once you have submitted your paperwork out web page will take you to a page that will allow you to click on a link to schedule your new patient appointment. If for some reason you are not taken to a scheduling page please text our office at 714-895-9355 because an error must of occurred.
Next you will have a consultation with Dr. Russell Reese to discuss your health related problems, concerns and potential treatment options. the initial visit is to allow Dr. Reese to learn more about you, your condition and expectations. During your visit you can ask questions and share your thoughts with Dr. Reese.
After your consultation Dr. Reese will perform a complete Chiropractic examination. However nothing will be done without your consent.
